Born and raised in Troy, Ohio, Milby is the youngest son of Filipina Elsie Lacia who hails from Surigao Philippines and American entrepreneur, Lloyd Milby.

Landed his big break in a toothpaste commercial in 2005 and later on became more famous as one of the house mates of "Pinoy Big Brother" (2005). His first media appearance was in the music video of "Just A Smile" by Barbie Almabis which was used in the promotion of that toothpaste product.

Was defeated by fellow house-mate Jayson Gainza in their one-on-one eviction in the first edition of "Pinoy Big Brother" (2005). House-mates nominated him for eviction since he "become too hard headed." However, it had been claimed that fans refused to vote for him to stay inside the house so he could exit sooner than he would want to be (he received 47 percent of the vote). When Sam exited the house, he was stunned by the number of fans that have followed his life inside the Big Brother house.

In 2006, he was launched as a member of all-male group Coverboys with John Lloyd Cruz, Zanjoe Marudo, Rafael Rosell, Victor Basa and Jake Cuenca.

With Piolo Pascual, he did a concert titled Heartthrobs at the Music Museum on February 2 and 3, 2007. It had its repeat on March 17, 2007 following the big success of the first concert.
